Plastic surgeons usually recommend removal of the implant if it has ruptured, even if the silicone is still enclosed within the scar tissue capsule, because the silicone gel may eventually leak into surrounding tissues. If you are considering the removal of an implant and the implantation of another one, be sure to discuss the benefits and risks with your doctor.

FDA completed a retrospective study on rupture of silicone gel-filled breast implants. This study was performed in Birmingham, Alabama and included women who had their first breast implant before 1988. Women with silicone gel-filled breast implants had a MRI examination of their breasts to determine the status of their current breast implants. The 344 women who received a MRI examination had a total of 687 implants. Of the 687 implants in the study, at least two of the three study radiologists agreed that 378 implants were ruptured (55%). This means that 69% of the 344 women had at least one ruptured breast implant. Of the 344 women, 73 (21%) had extracapsular silicone gel in one or both breasts.

Factors that were associated with rupture included increasing age of the implant, the implant manufacturer, and submuscular rather than subglandular location of the implant. Robinson et al. studied 300 women who had their implants for 1 to 25 years and had them removed for a variety of reasons. Visible signs of rupture in 51% of the women studied were found. Severe silicone leakage (silicone outside the implant without visible tears or holes) was seen in another 20%. Robinson et al. also noted that the chance of rupture increases as the implant ages. Other studies indicate that silicone may escape the capsule in 11-23% of rupture cases.

Capsular Contracture
The scar tissue or capsule that normally forms around the breast implant may tighten and squeeze the breast implant. This is called capsular contracture. Over several months to years, some women have had changes in breast shape, hardness, or pain as a result of this contraction. Although this seems to occur to some extent in most women with breast implants, there are no reliable data on how often this happens. If these conditions are severe, more surgery may be needed to correct or remove the breast implants.

Making Breast Cancer Harder to Find
The breast implant could interfere with finding breast cancer during mammography. It may "hide" suspiciouslooking patches of tissue in the breast, making it difficult to interpret results. The breast implant may also make it difficult to perform mammography. Since the breast is squeezed during mammography, it is possible for a breast implant to rupture during the procedure. It is essential that every woman who has a breast implant tell her mammography technologist before the procedure. The technologist can use special techniques to minimize the possibility of rupture and to get the best possible views of the breast tissue. Because more x-ray views are necessary with these special techniques, women with breast implants will receive more radiation than women without breast implants who receive a normal exam. However, the benefit of the mammogram in finding cancer outweighs the risk of additional x-rays.

Changes in sensation may result from breast implant surgery. These changes may be temporary or permanent. They may affect sexual response and the response of the nipple during breast feeding.

Shifting of the Breast Implant
Sometimes an implant may shift from its initial placement, giving the breasts an unnatural look. If the breast implant shifts, it may become possible to feel the breast implant through the skin. Other problems with appearance could include incorrect breast implant size, visible scars, uneven appearance, and wrinkling of the breast implant.

Unknown Risks
In addition to these known risks, there are unanswered questions about silicone-filled breast implants. For example, can the breast implants bring on symptoms of auto-immune diseases such as lupus, scleroderma, and rheumatoid arthritis? Can they bring on neurological symptoms similar to multiple sclerosis in some women? Can the breast implants increase the risk of cancer?

Connective Tissue and Related Disorders. These illnesses include autoimmune disorders such as lupus, scleroderma, and rheumatoid arthritis, as well as disorders such as fibromyalgia and chronic fatigue syndrome. Some women with breast implants have experienced these disorders as well as a variety of symptoms that could be related to the immune system. However, these symptoms may be present without breast implants or connective tissue disease. It is unclear at this time whether the signs and symptoms experienced by these women are related to their breast implants. In some cases, women have reported a reduction in symptoms after their breast implants were removed; in other cases, there was no change in symptoms after their breast implants were removed. Several human studies have been completed recently, which provide substantial, but not complete, information about any possible link between breast implants and immune-related disorders. These studies provide reassurance that the risk of developing a connective tissue disease due to breast implants is not high. Taken together, these studies tell us that the vast majority of women with breast implants will not develop defined immune-related disorders from their breast implants. Breast-Feeding and Children. At this time it is not known what effect breast implants have on lactation. Any breast surgery may impair breast feeding. A woman with breast implants who has questions about risks while pregnant or breast feeding should consult her physician. Cancer. Suicide Risk High in Women With Breast Implants

Women who get breast implants are three times more likely to commit suicide. The risk increases the longer the implants are kept.

This was revealed by Drs. Loren Lipworth and Joseph K. McLaughlin who said the suicides were not caused by silicone toxicity but underlying psychological problems. Their findings were published in the Annals of Plastic Surgery.

The subjects were 3,527 Swedish women who had breast implants for an average of 19 years. The risk of suicide went up 10 years after surgery and continued to rise after 20 or more years. The women were also three times more likely to have drug or alcohol problems and were more likely to be involved in an accident or injury as a result of these problems.

“What our data suggests to us is there is a subset of women choosing to get cosmetic breast implants who have psychiatric illness prior to implantation. This results in a high risk of unnatural cause of death – suicide and deaths related to alcohol dependence and drug abuse,” said Lipworth, an assistant professor of medicine at Vanderbilt University in Nashville, Tennessee.

Lipworth’s study is not the first to confirm the breast implant – suicide link. Dr. Louise A. Brinton, chief of the hormonal and reproductive epidemiology branch of the US National Cancer Institute, had established this link earlier and remains baffled why this happens.

“There could be a host of explanations. It could be these women have unrealistic expectations about how implants are going to change their lives. There could be complications of breast implants that affect the quality of life. Or there could be underlying personality predispositions that lead both to seeking implants and suicide. We can’t tell which, although anecdotal evidence points to the third possibility,” he told WebMD.

Atlanta plastic surgeon Dr. Foad Nahai, president of the American Society for Aesthetic Plastic Surgery, said breast augmentation normally results in improved self-esteem if it is done for the right reasons. However, he admits that not all plastic surgeons take time to screen patients properly. This view is shared by Dr. David B. Sarwer, associate professor of psychology at the University of Pennsylvania’s Center for Human Appearance.

“Plastic surgeons need to do a better job of screening patients for psychological illnesses. My colleagues at the University of Pennsylvania and I have recently reviewed these findings, and we argue that plastic surgeons should evaluate the psychological status of all patients. If they encounter a patient in psychiatric treatment or with a history of psychiatric hospitalization, they should insist that those patients get a mental health consultation prior to surgery,” Sarwer said.

“If a woman is getting breast implants to save a marriage or if they want to attract more men when they go into a singles bar, this isn’t going to work. But if a woman is doing it for herself, for her self-esteem, and wants to look better in her clothes, we are seeing someone who is doing it for the right reasons,” Nahai added.

breast reconstruction for one larger than other?

Hey, I had an accident when I was younger and now one of my breasts are larger than the other, id say 1-2 cup sizes difference. Im pretty sure my insurance would cover it but im scared to have an operation.
has anyone had a similar experiance?

Will they look and feel the same? (like one obviously has an implant, the other doesnt?)

Is an implant better than reduction? (one is like a b, other is a d)

What kind of implants (silicone,etc)

What does it feel like afterwards, how long to heal..

How do I know a good doctor from a bad one, or where to look up good doctors, or how can i choose one? My insurance doesnt cover many.

I can only tell you about my experience. It’s related, although not exactly what you’re asking about. These are just guidelines so that you can get an idea of a bad case scenario and be better informed, so you can make your own intelligent decision, based on what’s best for you

1) I had a mastectomy (complete removal of a breast), due to breast cancer.

About 1-1/2 years later, I had a tram flap breast reconstruction, from a plastic surgeon @ a very prestigious hospital here in the San Francisco Bay Area.

A tram flap breast reconstruction involves taking extra skin/fat layer from my stomach (like the extra spare tire that’s just ugly to look at anyway), and putting that on my chest, where the breast used to be. Plastic surgery shaped it just like the old breast that was taken away.

Sounded like a dream come true, I’d get my breast back and a tummy tuck to boot — whoo hoo!

The bad thing, was that less than 48 hours after the initial surgery, I was back in the OR because the surgery “didn’t take” and the tissue was dying off — like the entire breast.

This happened @ 3am, mind you, and I was awaken by a nurse who handed me a phone, and told me to “say goodbye to my husband”. There were nurses/orderlies rushing everywhere and I was unable to reach my husband. Both he and the answering machine picked-up at the same time, and the nurse grabbed the phone away from me, while muttering “i can’t find a vein” (because my veins have gotten really tiny from all the chemo i’ve been having). anyhoo, i felt a sharp prick on my neck, i saw blood shoot out into the sky, landing on my face and chest (i felt it was hot, i was surprised), and i passed out — all the while thinking that it was ok i didn’t reach my husband, i wasn’t ready to die that night.

to make a long story short, i had subsequent surgeries every 1-1/2 weeks for the next 2 months after that. a total of 6, i counted. each being more painful and traumatizing than the next.

the breast didn’t take. it was awful to see it die off every day. it was turning more and more black. they had to take it away finally or it would have taken me with it.

i have a scar that now runs from my left knee to my ankle, this is where they borrowed a vein to stick to my chest the first OR incident. i have a rectangular scar, about 6″ long and 4″ wide, going from the top of my bikini line, longwards toward my knee so I can never wear a miniskirt or shorts or a normal length swimsuit ever again. (i can only wear shorts over a swimsuit now, or a fake skirt, otherwise the hideous scar shows). i have a scar that runs the entire length of my stomach — no 2 piece bikinis for me. i have a hole below the scar, oval shaped, about 4″ at the widest. the skin on my chest where they attempted to place the breast is now soooo thin, that my rib cage is completely visible. it’s quite gross, really. and all this happened during Thanksgiving, through XMas and New Yar. Recovery was about 6 more months afterwards.

what else can i say? oh, make sure you do your research about your doctor. don’t trust that because he comes from a prestigous hospital doesn’t mean he’ll be conscientious about YOUR needs.

honestly, i felt that the since the guy is a plastic surgeon, the more surgeries he did (i had fabulous insurance at the time), the more money he and the hospital made. like he did surgeries that were not exactly necessary, that could have been done away with by other therapies. the guy didn’t tell me any of these worse case scenarios, i have come away from all this with more scars than i started with.

i just wanted to get my breast back. i thought that was easy and simple enough. i didn’t know i could almost die (3x this happened) from all the surgeries (which btw was to fix one problem from the previous surgery to another).

Should I get breast implants?

I am 22 years old and I am really unhappy with my body. I have been wanting breast implants for the longest time. I will have the money soon but I am VERY scared. I freak out when I just have to get a shot at the doctors. What should I expect? What will I go through and where should I get them done? I am from Portland, OR. But I think I might fly to California to get them done. I am very worried that they might look bad. I want them to look perfect and natural so I want good doctor. PLEASE HELP ME!!! I really want this, I am just very nervous.. I dont wanna regret this decision!
Right now I am a B almost a C.

I hope you’ve done your research on breast implants. I’m not going to scare you away, but you do realize you’ll have to get them redone once they start to get “old”. I’ve watched a lot of shows on it and I actually thought about it, but it’s too much money to blow on and the chances of it not going well are too high. Especially if you’re not going to someone who’s going to make you look perfect.

The breast can explode, the breast can start to harden after a while. Wouldn’t want chemicals in my body just so I can have more sex appeal. If you want a good doctor, you’re going to need a lot of money, especially in Cali. There are a lot of risk factors, in my opinion.
