Anyway, I was reading up on them and it says depending on which surgery is being performed–augmentation or reconstruction–the type of implant used will be determined. I would like saline implants, but apparently for augmentation surgery patients older than 22 they have to get silicone. I don’t know which surgery I will be having. I thought it was reconstruction, but now Im unsure. If I knew the difference I would be able to tell which I need and if I will be able to get the implants I want. Over several months to years, some women have had changes in breast shape, hardness, or pain as a result of this contraction. Although this seems to occur to some extent in most women with breast implants, there are no reliable data on how often this happens. If these conditions are severe, more surgery may be needed to correct or remove the breast implants.

It allows for more accurate positioning, a very important aesthetic consideration. The inframammary incision goes in or near the natural fold under the breast. It works well when the woman has enough tissue to have a distinct fold, but it typically is not a very good option when a defined fold is not present. This technique tends to preserve sensation well. It is the best option when old scar tissue (capsular tissue) requires removal in secondary surgery. Some women prefer not to have a scar on the white skin of the breast, even in the fold.

The transaxillary incision is placed within the axilla (armpit). It has the advantage of avoiding a scar on the breast. It has the disadvantage that if the scar does not heal well, it is visible when the arm is raised. It also has the disadvantage of making optimal positioning more difficult. Larger silicone gel implants may not be able to be placed via this approach. Secondary procedures are very difficult from the axilla.

The umbilical incision has been used by some because of the appeal of a distant incision. However, accurate positioning and pocket modifications are most difficult from the umbilicus. Silicone gels cannot be placed. Positioning issues have been described and most surgeons do not utilize this approach.

Each individual has unique concerns and priorities. The incision selection is just one part of the equation to a successful, pleasing result. This along with implant style, size and position and whether or not a lift is required are all issues which should be thoroughly discussed with your surgeon, to help arrive at an optimally aesthetic outcome.

Today, a variety of implant designs exist. It is now accepted that current implants require maintenance and will likely require replacement possibly multiple times in the course of a young woman’s life. These should not be thought of us permanent devices. More common reasons for replacement include deflation or rupture, hardening of scar tissue (capsular contracture) around the augmentation material and desire for change in size. Secondary procedures are often more involved than the initial one. The maintenance issue must be considered in making the decision to undergo implant placement.

All current implants have a silicone rubber shell. The shell can be filled with salt water (saline) or silicone gel. Because the body is largely composed of salt water, saline implants are the safest available. Saline-filled implants can be placed through smaller incisions because they are inflated after they are placed. However, because of their watery consistency, they are less natural than silicone gel implants that more closely resemble a breast in feel and appearance. One of the common complaints regarding saline is that particularly in thinner individuals, implant visibility such as rippling is greater. Usually over the course of years, the silicone rubber shell can crack, this results in deflation which requires replacement. The leaked saline is usually considered harmless and is absorbed and excreted by the body.

More recently, silicone gel implants have again become available for use in woman 22 years of age and older, who desire their placement. Patients and surgeons tend to agree that these look and feel more natural. As with saline devices, the silicone rubber shell may crack over time and rupture can develop. Fortunately, most ruptures are contained within the fibrous capsule that surrounds the implant. However, removal of the capsule and implant material is a more involved procedure than the initial augmentation procedure. Most ruptures beyond the capsules appear to have occurred in the past due to trauma related to the squeezing of the capsules (and implants) in an effort to soften firm scar tissue. This closed capsulotomy procedure is no longer a recommended procedure and it should not be done. Studies continue following FDA approval. It is hoped that the newer designs will have a lower rate of encapsulation (hardening), as well as a decreased rate of rupture. However, the studies are ongoing.

Newer materials include more "cohesive" silicone fillers. These materials could be less of an issue in the event of rupture. Silicone gel implants are often selected by very thin woman and those undergoing reconstruction of the breast following mastectomy.

Saline is the safest choice… Its basically salt water and if something were to happen (which is EXTREMELY rare) and they leaked it would not harm her in the least. Silicone on the other hand is VERY dangerous and could poison her if leaked into her body. They are said (by some doctors) to be more durable though… I would definitely choose the safer route though. If she wants a natural look but still a little dramatic have her ask about tear drop/high profile…

My aunt and close friends have had implants so I can speak for them. They have all had Silicone Implants and although they are more expensive, they are definitely worth it. With implants, you can’t afford to go cheap considering they will be there for awhile. So, I think you should just splurge and go for the silicone, you will be happier in the end. Good luck! :)

There have been varying viewpoints throughout the years about the best kind of implant to use for Breast Augmentation, and health controversies about one kind of implant that dominated news reports in the 1990’s.

Many women who were interested in having fuller breasts were fearful to do so because of these reports. The following describes the different kinds of implants that are available, and addresses some of the concerns regarding differences between breast implant fill material, shape, surface texture, size, manufacture, and use of implants.

Breast Implant Fill Material:
Although all breast implants do have a solid silicone shell (bag), they are filled with different substances such as: saline and silicone.

Saline Breast Implant
While there are a number of implants available for Breast Augmentation, currently the most commonly used is the saline breast implant.

The saline implant has an exterior sack made of solid silicone, which is filled with a sterile saline solution like the salt-water solution that is administered for intravenous fluids. One of the advantages of this solution is its safety. Should the implant for some reason leak, the saline just gets absorbed by your body, as would the intravenous fluids used during, and after surgery. Yet another advantage of the saline implants is a very low infection rate, which is usually less than one to two percent.

Another advantage of the saline implant is that it is filled with fluid after being inserted in the breast, therefore requiring only a small incision. This is in contrast to Silicone Implants which are pre-filled. Also, because saline implants are filled after insertion, their final volume can be adjusted making it easier to correct for existing breast asymmetry.

Widespread media reports that saline-filled implants can contract fungal contamination were discredited when it was learned that the Canadian laboratory that released this finding relied heavily on implants that had been mailed. As such, there was no guarantee of sterility in the transport of these implants.

There are two main disadvantages of Saline Breast Implants, when compared to those filled with silicone. Because of the liquid nature of saline, saline filled implants tend to have a higher incidence of rippling. Although rippling occurs in all saline implants, filling them according to the manufacturers recommendations, submuscular placement of implants, and the existence of thicker breast skin and tissue minimizes a persons ability to see or feel rippling of saline implants. Because silicone gel has a thicker consistency, it tends to ripple much less, but sometimes happens nonetheless.
