Breast Augmentation Incisions

Breast augmentation or breast enlargement is performed to enhance body proportions.

The success of the augmentation procedure relies upon a variety of decisions related to incision options, implant styles, sizes and positions. Incisions include peri-areolar (at the edge of the areola), transaxillary (within the armpit) and inframammary (in or near the breast fold). Each has associated advantages and disadvantages which must be weighed by each patient.

The peri-areolar incision has the advantage of being placed near a natural anatomical line (the junction of light breast skin with the darker areolar skin). It allows for more accurate positioning, a very important aesthetic consideration. Its disadvantage is that while most patients maintain normal nipple sensation, there may be a greater chance of lose of sensation.

The inframammary incision goes in or near the natural fold under the breast. It works well when the woman has enough tissue to have a distinct fold, but it typically is not a very good option when a defined fold is not present. This technique tends to preserve sensation well. It is the best option when old scar tissue (capsular tissue) requires removal in secondary surgery. Some women prefer not to have a scar on the white skin of the breast, even in the fold.

The transaxillary incision is placed within the axilla (armpit). It has the advantage of avoiding a scar on the breast. It has the disadvantage that if the scar does not heal well, it is visible when the arm is raised. It also has the disadvantage of making optimal positioning more difficult. Larger silicone gel implants may not be able to be placed via this approach. Secondary procedures are very difficult from the axilla.

The umbilical incision has been used by some because of the appeal of a distant incision. However, accurate positioning and pocket modifications are most difficult from the umbilicus. Silicone gels cannot be placed. Positioning issues have been described and most surgeons do not utilize this approach.

Each individual has unique concerns and priorities. The incision selection is just one part of the equation to a successful, pleasing result. This along with implant style, size and position and whether or not a lift is required are all issues which should be thoroughly discussed with your surgeon, to help arrive at an optimally aesthetic outcome.

Today, a variety of implant designs exist. It is now accepted that current implants require maintenance and will likely require replacement possibly multiple times in the course of a young woman’s life. These should not be thought of us permanent devices. More common reasons for replacement include deflation or rupture, hardening of scar tissue (capsular contracture) around the augmentation material and desire for change in size. Secondary procedures are often more involved than the initial one. The maintenance issue must be considered in making the decision to undergo implant placement.

All current implants have a silicone rubber shell. The shell can be filled with salt water (saline) or silicone gel. Because the body is largely composed of salt water, saline implants are the safest available. Saline-filled implants can be placed through smaller incisions because they are inflated after they are placed. However, because of their watery consistency, they are less natural than silicone gel implants that more closely resemble a breast in feel and appearance. One of the common complaints regarding saline is that particularly in thinner individuals, implant visibility such as rippling is greater. Usually over the course of years, the silicone rubber shell can crack, this results in deflation which requires replacement. The leaked saline is usually considered harmless and is absorbed and excreted by the body.

More recently, silicone gel implants have again become available for use in woman 22 years of age and older, who desire their placement. Patients and surgeons tend to agree that these look and feel more natural. As with saline devices, the silicone rubber shell may crack over time and rupture can develop. Fortunately, most ruptures are contained within the fibrous capsule that surrounds the implant. However, removal of the capsule and implant material is a more involved procedure than the initial augmentation procedure. Most ruptures beyond the capsules appear to have occurred in the past due to trauma related to the squeezing of the capsules (and implants) in an effort to soften firm scar tissue. This closed capsulotomy procedure is no longer a recommended procedure and it should not be done. Studies continue following FDA approval. It is hoped that the newer designs will have a lower rate of encapsulation (hardening), as well as a decreased rate of rupture. However, the studies are ongoing.

Newer materials include more "cohesive" silicone fillers. These materials could be less of an issue in the event of rupture. Silicone gel implants are often selected by very thin woman and those undergoing reconstruction of the breast following mastectomy.

Most Common Plastic Surgeries – How They Work and What They Cost

Plastic surgery has become increasingly popular in the United States. Overall, there are more of these procedures performed today than at any point in our nation’s history.

Here are the three most popular procedures, based on the number performed each year.

Cosmetic surgery, also referred to as plastic surgery, has become extremely popular in the United States. There are several reasons for this growth, ranging from social pressure to increased marketing by surgeons.

But it’s beyond the scope of this article to pass judgment for or against these procedures, or to speculate on the reasons for their popularity. Instead, I will list the three most popular cosmetic surgery procedures in the U.S., explain how they work, and give you an idea of what they cost. So here they are in no particular order.

Rhinoplasty

The Process – A quick lesson in word origin. "Rhino" comes from the Greek word for nose. "Plasty" refers to the act of molding or shaping something. Combine them and you have a process that reshapes the nose, commonly referred to as the nose job. To perform this procedure, the surgeon will make a small incision below the nose and extending into the nostrils. Through this, the surgeon can access (and reshape) certain areas of cartilage within the nose.

The Popularity – The nose job is a very popular form of cosmetic surgery in the U.S. According to the American Society of Plastic Surgeons (ASPS), there were nearly 285,000 performed in 2007.

The Cost – As with most plastic surgery procedures, the cost of a breast augmentation will vary based on several factors. The geographical location, the level of consumer demand, and the surgeon’s experience all play a role. Based on my research, the average cost of rhinoplasty was around $6,000, but could run as high as $12,000 in places with higher demand (think Beverly Hills and Los Angeles).

Breast Augmentation

The Process – Commonly referred to as the "boob job," breast augmentation is a straightforward procedure that uses synthetic implants to increase a woman’s breast size. The two types of implants currently being used in the U.S. are saline and silicon. Saline implants are regarded as the safer option of the two, while silicon implants are said to feel more natural.

 
The Popularity – According to the American Society of Plastic Surgeons (ASPS), there were roughly 212,000 breast augmentation procedures in 2000. Seven years later, in 2007, there were more than 347,000 of these surgeries performed. This is one of the fastest growing of all cosmetic surgeries.

The Cost – Here again, the cost of this procedure will vary based on different factors. After doing some price sampling from a dozen or so websites, I found that the cost ranges from $4,000 to $10,000. That includes all fees associated with the procedure, such as the cost of the implants, anesthesia, surgeon’s fee, etc.

Liposuction

The Process – Liposuction is a process through which a surgeon uses a suction device to remove unwanted fat. It can be performed in several areas of the body, most commonly in the abdomen, hips, thighs and buttocks. After making an incision in the area to be targeted, the surgeon uses a stainless steel tube known as a cannula to break up fat cells and suck out the fatty tissue.

The Popularity – The numbers have dropped a bit from 2000 to 2007, but it’s still a common type of cosmetic surgery. In 2007, more than 301,000 liposuction surgeries were performed in the United States.

The Cost – There are different forms of liposuction, and it can be performed on different parts of the body. Add to that the differences in consumer demand, surgeon availability and other factors, and you have a wide-ranging cost scale. Lipo on the abdominal area, for example, can range from $4,000 upward to $8,000.

Conclusion and Cautionary Statement

This article is not meant to trivialize plastic surgery, but only to chronicle its rising popularity within the United States. All surgeries have risks associated with them, and the items on this list are no different. Always do plenty of research and ask plenty of questions before making a decision about cosmetic or any other form of surgery.

 Breast Augmentation – Meeting Realistic Expectations In Breast Enhancement Surgery

Most would not be surprised to learn that breast augmentation or breast enhancement is one of the most commonly requested cosmetic surgical procedures in the United States.

The purpose of the operation is quite direct. It is performed to improve the appearance of the breasts. In terms of efficacy and safety the procedure fulfills its objectives. There are several reasons a woman may desire this procedure. It can be used to enlarge small and underdeveloped breasts or to essentially re-inflated breasts that have shrunk down following weight loss and pregnancy. A lifting effect is one additional benefit of breast augmentation. This is particularly beneficial when following pregnancy when there is a decreased volume as well as sagging.

Every woman who considered breast augmentation must make a decision if the operation, with all of its inherent risks and benefits are correct for her. Each woman will come to a different conclusion and will, of course, follow a different path. She must have a good knowledge of her goals and expectations. She must be truthful to herself when considering all of the lifetime implications of the procedure. A thorough examination with a plastic surgeon will be helpful to determine how the breasts conform to the so called normal standards. This includes breast shape, the degree of sagging if present, the base diameter of the breast, location of the nipple, size of the areola, symmetry of size and shape as well as the level of the crease under the breast.

There are several key points to consider for each individual. Will the implant be in proportion to the rest of your body? Body symmetry and proportion are quite important considerations. Is the choice of incision appropriate to your desires and can it be well hidden considering the types of clothing and bathing suits you like to wear? There are many choices when it comes to incisions and a thorough discussion will help you decide the correct location for you. This will not be the same for all women. Have you considered the different shapes of the implant styles? There is now a wide range of implant shapes and sizes available as well as different profiles. This allows for improved accuracy when it comes to meeting individual desires. You must discuss the desired shape and then the appropriate implant can be selected. Is the implant to be placed above or below the muscle? There is new evidence that using the third and fourth generation implants above the muscle has a lower rate of capsular contracture than with the previous generation of implants. Placing the implant above the muscle is a growing trend among plastic surgeons in the United States.

It is important that each woman consider other factors such as the amount of time needed for recovery, the possible loss of nipple or breast sensitivity and the possible loss of the ability to breast feed. There are also other issues to consider such as the use of a silicone gel implant versus a saline filled implant.

Breast augmentation is a safe procedure that, under the correct circumstances with give the expect result. It is important that the procedure is performed for the right realistic reasons and that the patient is emotionally stable. Every woman who is considering this operation will benefit from doing their own research. It is from diligently educating themselves about the options that a good decision can be made.
