Actually there is a third option, which is silicone cohesive gel. This is silicone but it has a ‘gummy bear’ consistency so if the implant shell ruptures, the implant will not leak. These implants keep their shape and there are several types to choose from so you can choose how much prejection you want and how wide you want them. The drawbacks are that they are slightly more expensive than regular silicone and saline, the operation time is a bit longer and they require a larger incision than regular silicone or saline implants.

If you do go for the silicone cohesive gel, I would go for the antomical shape placed behind the muscle as this will look the most natural. Also going behind the muscle will minimise the chance of capsular contracture, which is a hardenening of the scar tissue around the implant that can cause the breasts to become overly firm.
